Dental Benefits

Program

The CSU offers two types of dental plans, a managed fee-for-service plan and a prepaid plan. Delta Premier Dental the CSUs managed fee-for-service dental plan. Delta Care (PMI) the CSU's prepaid dental plan. Provisions of Collective Bargaining Agreements may modify dental plan benefits. At this time, the employer pays the monthly premiums for both dental plans.

Eligibility

Employees must be hired at half time (or more) in an appointment that exceeds six months (an academic year for faculty members). It is the employees responsibility to request additions, deletions, or changes in their enrollment in a timely manner and to stay informed about the eligibility requirements.

Delta Dental Premier (Group #4018)

Delta Premier Dental plan is a group dental plan that allows you to choose any dentist, but the benefits described in their Evidence of Coverage and Summary brochure are guaranteed only when you go to member dentist. Your position determines the level of coverage (Basic, Enhanced Level 1, or Enhanced Level 2) you are entitled.

Identification cards are not issued. Claim forms for reimbursement are required. Forms may be obtained at offices of dentists who are Delta members, or by contacting Delta Dental directly at (888) 335-8227. The group number should be identified on all claim forms submitted to Delta Dental.

DeltaCare (PMI)

Delta Care is a prepaid plan; many services are covered at no cost to the employee, while other services have copayments for certain procedures. Your position determines which level of coverage, Basic or Enhanced, you are entitled. Under the Basic Plan, you must select one dentist who will provide all dental care for you and your family. Under the Enhanced Plan, you and your dependents have the option to select a maximum of three separate dentists to meet individual needs within your family. At the time of enrollment, you must choose a DeltaCare provider.

Once you have enrolled, you will receive a DeltaCare membership card and an Evidence of Coverage that fully describes the covered benefits. The membership card will have the telephone number and address of your selected dentist. Enrollees can request to change their plan dentist by contacting Delta Care (PMI) at (800) 422-4234.

FlexCash Plan

FlexCash is an optional benefit plan that allows you to waive medical and/or dental insurance plan(s) in exchange for cash if you have other non-CSU coverage. You may enroll in FlexCash within 60 days of eligibility or a change in family status as defined by the IRS; OR during Open Enrollment (or within 60 days of a return from a leave of absence that was during the regular open enrollment period).

Enrollment

To enroll, you must complete the Dental Enrollment Form STD 692 at the Benefits Services Department. The deadline to enroll is 60 calendar days from date of eligibility. You may begin to use your dental plan when the deduction description Delta II or PMI 2 appears on your pay warrant stub. If you are enrolling a spouse, their social security number is required for processing. Please have this information available when enrolling.

Open Enrollment

If you do not enroll by the 60th calendar day of eligibility or you wish to change dental plans, you may do so during the annual open enrollment period.

Cancellation/Termination/Cobra

You may elect to voluntarily cancel your coverage at any time. If you become ineligible because of a change in employee status or timebase, or your dependent(s) becomes ineligible, coverage may be continued under the provisions of COBRA. Direct Pay

If you are on a Leave of Absence Without Pay or are otherwise in an approved non-pay status, you may elect to continue your dental coverage by paying the full premium for your coverage via the Direct Pay method.
