The "As of" date for your Marital Status does not necessarily reflect the date your status changed (or the date you were married.) If you were a CSULB employee when we went 'live' in PeopleSoft Human Resources, the date shown may be as of the date we converted your data from our legacy system. The default value for marital status is single. So, in some cases, you may not be single, but because you do not have a spouse enrolled in your health care plan, we have no reason to know your marital status. If you need to have any information updated changed, please contact the Employee Self Service Help Desk at firstname.lastname@example.org or 985-7393.
The Other Personal Information that displays is confidential demographic data you provided to us at the time of your initial hire and sign-in at Payroll. If any of this information is incorrect, please contact the Employee Self Service Help Desk at email@example.com or 985-7393. You will be given further instructions. Do not email confidential information changes. You may be required to provide proof of the change and or complete a form at the Payroll & Benefits Services office.
The Smoker Status field defaults as "Non Smoker" and we do not maintain this data for health benefit or other purposes.
Birth Country, Birth State may be blank unless you were first a student of CSULB before you became an employee.
Date Entitled to Medicare is purposely left blank.
