Open Enrollment 2009
(September 14 – October 12, 2009)
To make changes during Open Enrollment submit open enrollment forms September 14, 2009 – October 12, 2009 to Benefits Services located in Brotman Hall 353 at the Payroll/Benefits Service Counter. You may access our open enrollment forms.
Note: If you are making any type of health enrollment changes (i.e. plan changes, add/deleting dependents to your health or dental plans) you will be contacted by our office to sign final documentation to complete your request.
There are some important changes to your health benefits rates effective January 1, 2010. Be sure to review this information carefully to determine whether you want or need to make a health plan change.
Blue Shield HMO Access+ monthly deductions will decrease to the following rates:
- Employee Only = $24.09 Unit 6 Employee Only = $19.09
- Employee + 1 = $98.18 Unit 6 Employee + 1 = $88.18
- Family Coverage = $142.43 Unit 6 Family Coverage = $122.43
Kaiser HMO monthly deductions will increase to the following rates:
- Employee Only = $1.99 Unit 6 Employee Only = no charge
- Employee +1 = $53.98 Unit 6 Employee + 1 = $43.98
- Family Coverage = $84.97 Unit 6 Family Coverage = $64.97
PERSCare monthly deductions will increase to the following rates:
- Employee Only = $338.50 Unit 6 Employee Only = $333.50
- Employee + 1 = $727.00 Unit 6 Employee +1 = $717.00
- Family coverage = $959.90 Unit 6 Family Coverage = $939.90
PERSChoice monthly deductions will decrease to the following rates:
- Employee + 1 = $38.50 Unit 6 Employee + 1 = $28.50
- Family Coverage = $64.85 Unit 6 Family Coverage = $44.85
No employee monthly deductions will come out of your check if you are enrolled in Blue Shield NetValue, PERS Select or PORAC (Unit 8 Employees Only).
