Selecting A Health Plan Provider

If you are enrolled in an HMO, with the exception of Kaiser, you must select a primary care physician, IPA group or medical facility. Brochures and directories for each health carrier are available in the Benefits Services Department.

We recommend you visit the carrier's website for most current information.

If you would like to change your medical group or provider, please contact your current health plan. If you would like to change your medical carrier (Blue Shield HMO, Kaiser, PERSCare, PERSChoice) you must make your change during the open enrollment period or within 60 days from retirement, or move (address change) from your current providers covered area.
