Affiliation Agreements
Definition
An Affiliation Agreement is a contract between the university and a facility where university departments want to send university students for course-related and graduate required off-campus fieldwork experience (Internship). The following departments currently participate in this program:
- Biological Sciences/Phlebotomy
- Family & Consumer Sciences
- Electrical Engineering (Biomedical Engineering)
- Health Care Administration
- Health Science
- Kinesiology & Physical Education
- Physical Therapy
- Psychology
- Nursing
- Radiation Therapy
- Recreation & Leisure Studies /Recreational Therapy
- Social Work
- Speech Pathology and Audiology
(previously, Communicative Disorders)
- Student Teaching
Agreement Process
The process for putting an agreement in place takes two forms. The University prefers to use its agreement, but hospitals and a few other facilities are bound to use the one prepared by their organization.
- Request for Agreement. An agreement is initiated by a written request from a department -- verbal information is never used. Fax and e-mail are both acceptable. The request must include the basic information such as the facility name, address, phone and contact.
- Generate New Agreement. Terms and conditions of an agreement can be negotiated at any time during this process by the affiliation agreement coordinator. Contact coordinator for further information.

Numbering System
An eleven-character (including hyphens) number identifying the following information: Department identification, and consecutive number month and year of contract start. Here are some examples:
