Network Services Forms

Get Acrobat Reader

You will need one of these forms to get a dial-up, NT/LAN, or Lotus Notes account. Fill it out, sign it, and submit an original copy of the application form with a readable copy of your staff ID card to:

Computer Account Application form

This form can be used to obtain:

  • an Internet Account. A dial-up account offers E-mail, Internet service and dial-up access. Submit application to ITS Office with a valid University staff ID.
  • a Domain Account. Technical coordinators, use same form as DAF Notes account.
  • a DAF Lotus Notes Account.
