Section 5

This information is required when installing a new service or moving an existing service in order to maintain the accuracy of 911 system information. Change activities may also need this portion of the work order to be completed if the changes are being performed to better suit a new user. The new user's name, title, email address, or department should be updated in the extension record in order to maintain accuracy in database records for the campus directory, the 911 system, and for campus operator use.
