
In general, there are four different types of work order activities:
For installations in a new building, multiple or departmental moves, contact the Network & Telecommunications office at extension 54480 for voice and data project forms. Please allow ample time for the completion of any installation or other requested activity.

When installing a new telephone set or line for a fax, modem, or alarm, enter the room number and the building name or number where the service needs to be installed as shown below. Attach a floor plan of the room with a triangle pointing to the outlet to be utilized or newly installed. Floor plans are required for all installations and moves or the work order will be returned to the contact. If installing a local point to point data connection, indicate the originating and terminating points of the circuit, and include a floor plan of both points (with data jack numbers). A minimum of directory information, such as name and department, is required for all voice services to support the 911 system. If partial information for a non-published number, or complete information for a published number, is not included the work order will be returned to the contact. Convey any other instructions on the lines provided. Be sure to indicate the telephone type, inbound and outbound access in the section called Service Groups as indicated in the "NOTE" below the building and room number.

A telephone work order is required to have an extension and/or device removed from service and the monthly telephone bill. Please do not unplug a telephone from the jack, or plug a telephone into a jack other than the one assigned to it. A technician representing telecommunications will be dispatched to retrieve any devices that have been disconnected.

Changes in service normally consist of software changes such as: Authorization Codes, Programmable Keys, Feature Options (voice mail, hunt groups, pick up groups, etc.), Outbound Dialing Access (a change in Inbound Dialing Access requires an extension number change). Service Options related to adding a new TAU digital data device to an existing digital set is considered a change, but Data Communications installations and changes will be accepted under Install New Service or Change Existing Service activities. Convey any other instructions on the lines provided.

Enter the room number and the building name or number on the lines indicated. Attach a floor plan of the room with a triangle pointing to the location of the outlet to be utilized. Floor plans are required for all moves, or the work order will be returned to the contact. If moving a data connection, include a floor plan of originating and terminating points, and give as much information as possible on the lines for additional instructions. Please include the data jack numbers and any network identification numbers that may be available. Provide appropriate directory information and convey any other instructions on the lines provided.
