A Certificate of Insurance (COI) is a document that provides evidence that the University maintains appropriate insurance at the levels required by an Agreement. A Certificate of Insurance does not extend insurance coverage to a third party.
A Certificate of Insurance (COI) with an Additional Insured Endorsement provides evidence that the University maintains appropriate insurance at the level required by an Agreement and extends insurance coverage to a third party.
The campus office of Safety and Risk Management can provide a Certificate of Insurance to a third party when an Agreement has been properly executed between the University and the third party, and a COI is required. A Request for a Certificate of Insurance must be completed by the campus individual requesting the COI and provided to Safety and Risk Management at least 5 working days before the COI is needed.
If a Certificate of Insurance with an Additional Insured Endorsement is requested, a copy of the signed Agreement must be forwarded with the request. As an Agreement is a legally binding document that commits the University to explicit terms and conditions, it is of the utmost importance that the terms and conditions of these Agreements be reviewed for legal requirements, conformance to CSU and CSULB policy, and be reflective of good business and risk management practices. The campus Purchasing Office has responsibility for this review. Safety and Risk Management will not process a request for a Certificate of Insurance with an Additional Insured Endorsement unless the Agreement has been signed by the Director of Procurement and Support Services or his designee.
